Stephan Ames, MD
Dr. Ames earned his medical degree in 1987 from Oregon Health Sciences University, here he was a member of Alpha Omega Alpha Honor Society. He has a special interest in newborns, children, young adults, dermatology, asthma patients and preventive medicine. He is the Medical Director for Marquis Assisted Living. He has a special rapport with our elderly patients.
Michelle Kaplan, MD
Dr. Kaplan earned her medical degree in 2001 from the University of Southern California, Los Angeles. Her areas of interest include pediatrics and women’s health. She also has a special interest in asthma care. Dr. Kaplan is fluent in Spanish.
Kathleen Jackson, MD
Dr. Jackson earned her medical degree in 1988 from the University of Kentucky and has practiced Internal Medicine in Lane County since 1991. Her special interests include high blood pressure that is difficult to control, congestive heart failure and a broad spectrum of adult health conditions.
Mallory Crownover DNP, FNP-BC
Mallory received her Doctorate of Nursing Practice degree from the University of Florida. Before joining Thurston Medical Clinic, Mallory worked for the Florida Department of Health. As a Nurse Practitioner, Mallory offers patient centered care with emphasis on healthy lifestyles and illness prevention.

Our schedules are always busy, however, if you call and you feel you should be seen prior to the earliest available appointment, let the receptionist know. We generally have same day appointments. Remember to call to schedule your annual physical far enough in advance to accommodate your needs.
In the event you need to refill a prescription, call your pharmacy even if you have no more refills left on your prescription. PLEASE ALLOW 24-48 HOURS FOR AN APPROVAL. Also, please request your prescription refills Monday – Friday, as weekend prescriptions are usually not considered an emergency.
NO NARCOTIC prescriptions will be filled after hours and on weekends.
It generally takes several days for non-emergency test results to return to the office. We will contact you at our earliest availability once your doctor has reviewed them. Labs are processed through Interpath Labs unless otherwise advised.
In the event you have a medical question or concern, call our office and request your doctor’s assistant to call you. The receptionist will need to take a brief detailed message. Our assistants make return phone calls in between the scheduled patients (when possible), and generally prior to their lunch hour or going home for the evening. Remember, they are assisting the physician with regularly scheduled patients and will return your call when possible. If needed, leave the best time to call you back with your message. For non-emergency questions you can send a message through the Patient Portal to the doctor’s assistant.
Evenings and Weekends
One of our physicians is on call 24 hours a day. If you have an urgent matter you feel cannot wait until the next day; CALL OUR OFFICE TELEPHONE NUMBER. Follow the recorded instructions. An operator will come on the line to take a message and page the on-call doctor. GENERALLY the doctor will get back to you within the hour and will respond to all urgent telephone calls. If your question is not an urgent matter, please wait until the following day when one of our providers or their medical assistant can help you.
We encourage you to speak with our Patient Account Specialist if you have any billing questions or need to make payment arrangements. Call the business office at. If you have specific questions regarding your insurance coverage refer to your insurance member handbook or call the number provided on your insurance card.

New Patient Packet
Please print and fill out the New Patient Packet information. You can fax or mail them to the office or you can come 20-30 minutes early to your appointment and fill them out here. Please make sure to have an accurate list of your medications and doses.
You will need to bring to your appointment photo identification and your insurance card.
